What you need to bring

Have these ready so we can complete your affidavit in one session.

  • Valid government photo ID — driver's licence, passport or provincial photo card. The name must match the affidavit.
  • The completed, unsigned affidavit. We commission it; we don't draft it or give legal advice on its contents.
  • Any exhibits referred to in the affidavit, in order. Each exhibit is stamped and signed by the commissioner.
  • Know whether you'll swear or affirm. Swearing is a religious oath; affirming is a solemn promise. Both are equally binding.
  • An interpreter, if needed. You must understand the affidavit. The interpreter also swears an oath and must attend.

How your appointment works

  1. 1

    Identity confirmed

We check your photo ID against the name on the affidavit.

  2. 2

    Document reviewed

We confirm it's complete and that you understand it.

  3. 3

    Oath or affirmation

You swear or affirm that the contents are true, then sign.

  4. 4

    Commissioned

We complete the jurat, mark exhibits and apply our stamp.
